Dr. Rey has lost it. His looks have tanked. The good in his marriage has gone bad. His body (which is shown too often) is unpleasant to see. His charm is dead. Back in the day he did have a youthful bit of zeal that was enamoring, but now it's evident that living in excess has caught up to him. In order to pump up the show for high ratings, they have had to engineer fights and conflicts. All of the doctors are now so full of pride and are living in such excess that it is uncomfortable to watch them. There is so much focus on the fake drama that there isn't enough time to see the transformations of the surgery patients. Honestly, it's bragging and one ups-man-ship so poorly contrived that it is impossible to watch.

It's basically Dr Ray and his wife arguing... super awkward.
